Application Submission Instructions

An electronic copy of your PHS Form 5161-1 grant application and application cover sheet (Word) should be submitted to your respective Association no later than 8 p.m. ET on July 23, 2013.

The associations may request as many additional copies as they require and retain those copies for their files.

Applicants should follow the PHS 5161-1 application instructions except as otherwise noted herein for content and formatting of their applications. For further assistance with the application forms, each applicant should contact their member association. Project plans should address activities to be conducted over the entire project period.

The applications should be written as follows:

  • 30 page limit. The 30 page limit is for the narrative, it is exclusive of the letters of support and the appendices.
  • Times New Roman 12-point single space and single sided.
  • 1 inch margin (top, bottom, left and right).
  • Executive Order 12372 does not apply to this program, so therefore it is not subject to review.
  • Use of the modular budget format is prohibited when submitting any applications to AACN, APTR, AAMC, or ASPPH. All applicants must submit the detailed budget pages.

Application Review Information

The review and scoring criteria will be included in each WIP description provided by CDC. The criteria may differ among WIPs.


If applications submitted for review are considered incomplete or non-responsive, they will not be entered into the review process, and applicants will be notified that their applications did not meet the submission requirements. The following criteria will be used to determine responsiveness:

  • Applications must be received by the Application Deadline Date referenced herein.
  • As stated in the WIP, the applications must not exceed the Approximate Average per Award funding.
  • Proposed project must not exceed the "Project Period Length."
  • Applicants must meet the requirements defined "Special Eligibility Requirements" for each WIP, if any.

Award Notices

Successful applicants will receive a notice of award and reviewers' critiques in September 2013 from their member association. Each unsuccessful applicant will receive the reviewers' critiques of their application.

CDC does not permit direct contact between the Centers, Institutes and Offices (CIOs) and applicants. Applicants must address all questions to their member associations who will forward them to CDC. All questions will be posted on the FAQ page as they are forwarded to CDC. CDC's response will also be posted.

All questions pertaining to the WIPs will be addressed through July 16, 2013. No additional questions will be addressed after the deadline date.
