Formatting Guidelines

Note: An original and six collated copies of your full grant application and appendices must be submitted to your respective association by 5:00 p.m. EST on March 14, 2008. Please be sure to read the nonresearch application review criterion prior to preparing your application. |
The following PHS 5161 (Revised 7/00) Grant Application must be submitted for consideration for all non-research PEP applications and Investigator-initiated applications. Applicants should follow the application instructions noted below. The original and six copies of all application materials must be collated.
Cover Sheet: The cover sheet will be given to you by your respective association after submission of your Letter of Intent.
Face page (SF 424): Must be signed by an Authorized Representative from the school/university. In addition: Section 2 - The Applicant Identifier is the PEP number or IIA (for investigator initiated applications) and the application ID code that you receive during the LOI cycle from your Association. Section 10 - The Catalog of Federal Domestic Assistance Number is 93.283
Budget Information - Non-Construction Programs (SF 424-A): Fill out pages 1 and 2 for your entire proposed project period. Do not fill out the Budget Information - Construction Programs.
Assurances - Non-Construction Programs (SF 424-B): The form must be signed by a duly authorized official of the applicant organization. Do not fill out the Assurances-Construction Programs form.
Certifications: Pages 17 - 19 must be signed by a duly authorized official of the applicant organization.
Program Narrative: PHS 5161 has a 30 page limitation. The 30 page limit is for the narrative, it is exclusive of the letters of support and the appendices. The program narrative should be prepared using Courier new 12-point unreduced font, and written in plain language (avoiding jargon and undefined acronyms).
Budget Narrative: Provide budget justification for direct costs.
Checklist: Complete pages 25-26.
College and University Rate Agreement: You must include your institution's federally negotiated rate agreement. Please provide six collated copies with your application.
Appendices - Provide an original plus six collated copies.
