Question:

Where can I obtain the PHS 5161 grant application?

Answer:

The PHS 5161 grant application can be obtained from the Center for Disease Control and Prevention (CDC) Web site at www.cdc.gov/od/pgo/funding/forms.htm. The fillable forms are available on this site as a Microsoft Word document.

Question:

What is the page limit for the research plan? Does that include letters of support and appendices or is the 30 page limit just for the narrative?

Answer:

The page limitation is 30 for the PHS 5161. The 30 page limit is only for the project narrative and budget. Letters of support and appendices are not included in the page limitation.

Question:

What is the font size requirement? Margins? Single or double spaced?

Answer:

The program narrative should be prepared using Courier new 12-point unreduced font, and written in plain language (avoiding jargon and undefined acronyms).

Question:

For Question 16 - Will these applications be subject to review by state executive order process?

Answer:

Executive Order 12372 does not apply to this program.

Question:

For Question 18 - Who would be the authorized representative? Will this person also sign the Assurances portion of the application?

Answer:

The applicant needs to check with their grants/business office to find out who the authorized representative is for their organization and who is authorized to sign off on grant applications.

Question:

What needs to be submitted for the Executive Order- would this include the letters of support?

Answer:

This program is not covered by the Executive Order 12372.

Question:

On the Checklist page of the PHS 5161; where is states, "Has a Public Health System Impact Statement for the proposed program/project been completed and distributed as required?" ...I understand that the Public Health System Impact Statement is part of the narrative...however, I am not clear as to direction on the "distribution" part. Also, please clarify the Public Burden Statement on the same page and who this is applicable to; I think they are speaking to the reviewers? Please advise.

Answer:

The following information is taken from the PHS Policy Statement:

Public Health System Reporting Requirements:

Community-based, nongovernmental organizations applying for health services grants will be notified in the application materials if the program is subject to the Public Health System Reporting Requirements. Such applicants shall submit a copy of the application face page (SF-424) and a one-page summary of the project, called the Public Health System Impact Statement (PHSIS), to the heads of the appropriate State and local health agencies (health department, department of mental health, department of hygiene, etc.), as determined by the applicant, no later than the Federal application receipt deadline date.

The PHSIS and SF-424 application face page must be submitted in conjunction with all new and competing continuation applications and the first noncompeting continuation application in the initial project period or any extension thereof. The PHSIS must be no more than one page in length and shall address the extent to which a proposed project affects, and is related to, existing community services.

The PHSIS shall include the following information, which may be taken from the application's Program Narrative:

A description of the population to be served whose needs would be met under the proposal A summary of the services to be provided A description of any coordination planned with the appropriate State or local health agency(ies) The Public Burden Statement informs applicants of the average time it should take to gather the information asked for on the Checklist and that a person is not required to respond to a collection of information unless it displays a currently valid OMB control number. The OMB control number for this form is: OMB Approval No: 0920-0428.
